The controller, WALSHA – MOLENSTRAAT 280, 2490 BALEN respects the Belgian law of December 8, 1992 regarding the protection of privacy in the processing of personal data.

 

WALSHA only provides your personal data to third parties if necessary for the execution of the concluded agreement with you, or to comply with a legal obligation.

  • For processing and supporting your application
  • Financial handling and accounting processing
  • the permission to the staff of WALSHA, in function of observation, diagnosis, counseling and/or treatment of the above-mentioned client/patient, to enter into communication with, and/or request file data from and exchange with third parties and/or to take digital images as part of a diagnostic observation.
  • the permission to the employees of WALSHA, to prepare, report on legal files and share them with the relevant departments, after review and consent of the undersigned
  • Permission to take a digital file photo of the client

 

If the parents of a minor client live separated legally or by mutual consent, under any arrangement towards the children, the parent, declares to inform the other party about this counseling and share the coverage.

 

The data obtained and collected may be processed by WALSHA and stored physically and/or electronically, according to, in accordance with the General Data Protection Regulation.

 

In certain cases, employees also have (limited) access to certain personal data, for example, in the case of registrations, appointment recording, general file management and filing. They too, are bound by the duty of discretion and have no more access to your data than is strictly necessary to perform their duties.

 

Your records, according to legal requirements, must be kept for 30 years. Other information that does not belong to your file is kept according to the time limits established by law or no longer than necessary to fulfill the purpose of processing.

 

The data we process from you will be shared only as part of your care, with other health care providers or service providers with whom you have a treatment relationship, or otherwise shared anonymously. Only this information is shared that is necessary for your treatment/follow-up by another health care provider/service provider.

 

In the context of legal obligations such as reimbursements by the NIHDI and/or reimbursements (from supplementary insurance) of assistance provided by insurance institutions/hospital funds, certain data are also shared with them, as well as for non-insured patients at the expense of the CPAS.

 

You have a legal right to inspect and possibly correct your personal data. Provided proof of identity (copy of identity card), you can obtain the written communication of your personal data free of charge via a written, dated and signed request to WALSHA, MOLENSTRAAT 280, 2490 BALEN, ONTHAAL@WALSHA.BE. If necessary, you can also ask to correct the data that would be incorrect, incomplete or not pertinent.
